ABSTRACT

What can be the contribution of oral history to the interpretation of tangible cultural assets? Starting from this conceptual question, this article focuses on the case study of the experiences Second World War in Naples bomb shelters, recently included within the Underground Built Heritage (UBH) class. The hypothesis of the research is that bomb shelters are very significant elements in the subsoil of Naples but that, due to the lack of distinctive elements and dedicated storytelling, they are only partially exploited in the context of urban parks or generic itineraries Naples's subsoil. The thesis of the research is that the memories of those children that took refuge there during World War II (WWII), which were collected with the adoption of the oral history methodology, can integrate their value as elements of local cultural heritage and eventually support their interpretation for the benefit of the new generations. The methodology adopted was the collection, via structured and unstructured interviews, of the direct testimonies of those who took refuge in Naples' underground during the alarms. Twenty-three interviews were carried out, and all the issues introduced have been classified according to the various themes addressed during the narration in order to allow the reconstruction of dedicated storytelling in the future. The research was carried out immediately after the acute phase of the COVID-19 pandemic, an event that claimed many victims belonging to the generation of our witnesses, whose memories were at risk of being lost forever. © 2023 by the author.

ABSTRACT

In this article, the challenges that cultural and creative industry (CCI) firms face in forming sustainable business models—issues heightened by the ongoing COVID-19 pandemic and the evolving consumer patterns that it has triggered—are addressed. The goal of this study was to identify a set of unique key elements crucial for the construction of a business model that aligns with the distinct characteristics of CCIs. To achieve this goal, an in-depth, long-term study using semi-structured interviews with proprietors in Eastern Taiwan was conducted. This region is home to unique, small-scale cultural and creative businesses and represents less than 5% of Taiwan's population. The semi-structured, in-depth interviews served as the data collection method, while content analysis was used for data interpretation. This approach allowed the current study to encapsulate a set of key elements that could inform the creation of a business model for CCI firms. The insights gathered by the study provide a robust framework for the development of sustainable CCI business models, offering valuable guidance for both existing businesses seeking to adapt and grow and new entrepreneurs entering the industry. This research also aims to stimulate further scholarly debate on the importance of a tailored, multidisciplinary business model for CCIs, given their unique needs and characteristics.

ABSTRACT

The aim of the present research was to develop and test the efficacy of a novel online contingent attention training (i.e., OCAT) to modify attention and interpretation biases, improve emotion regulation, and reduce emotional symptom levels in the face of major stressors. Two proof-of-principle studies were carried out. In study 1, 64 undergraduates who were about to start a major stressful period (i.e., final exams) were randomized to undergo 10 days of active OCAT or a sham-control training. Emotion regulation (habitual use of rumination and reappraisal) and symptom levels (depression and anxiety) were assessed before and after the intervention. In study 2, the same 2 × 2 mixed design was used with 58 individuals from the general population undergoing a major stressful situation (the lockdown period at the beginning of the COVID-19 pandemic in 2020). In both studies, the OCAT group showed significant improvements on attention towards negative information and interpretation biases in comparison to the sham-control group. Additionally, changes in cognitive biases transferred to reductions of participants' use of rumination and anxiety symptom levels. These results show preliminary evidence regarding the efficacy of the OCAT to target attention and interpretation biases as well as to improve emotion regulation processes and to buffer against the effects of major stressors.

ABSTRACT

AIM: As the United States population is ever changing and is growing in diverse population patterns, the health care system is called to initiate responsive health care practices that are based on the public's changing and diverse cultural patterns. This study sought to explore the perceptions of certified medical interpreter dual-role nurses and their experiences with Spanish-speaking patients from admission to discharge in hospital stays. DESIGN: A qualitative descriptive case study was applied in this study. METHODS: Data was collected from nurses working at a United States Southwest Borderland hospital using purposive sampling and semi-structured in-depth interviews. A total of four dual-role nurses participated, and thematic narrative analysis was applied. RESULTS: Four major themes emerged. The main themes were "being a dual-role nurse interpreter," "patient experience," "culture and competence, "and "nursing and caring," With each major theme multiple sub themes emerged. Two sub themes emerged with "being a dual-role nurse interpreter," and two sub themes emerged with "patient experiences." The major themes that emerged from the interviews indicated that the language barrier greatly affects Spanish-speaking patients in their hospital stay. Participants reported having at least one encounter with a Spanish-speaking patient that was not afforded interpretation services or had interpretation from someone other than a qualified interpreter. Patients experienced confusion, apprehension and anger associated with not being able to communicate their needs to the healthcare system. CONCLUSIONS: According to the experiences of the certified dual-role nurse interpreters, having a language barrier makes a tremendous impact on the care of Spanish-speaking patients. Nurse participants describe how patients and their family members experience dissatisfaction, anger and confusion when there is a language barrier and most importantly how language barriers have detrimental effects on patients with wrong medication prescriptions and wrong diagnosis. CLINICAL RELEVANCE: When hospital administration recognize and support nurses as certified medical interpreters as a key component for patient care when assisting persons with limited English proficiency, patients are empowered to become active members of their healthcare regimen. The role of dual-role nurses enables brokering between the healthcare system and serves as a tool to bridge health disparities based on linguistic inequities existing in healthcare. Recruitment and retention of certified medical interpreter trained Spanish-speaking nurses deter errors in healthcare and makes a positive impact on the healthcare regimen of Spanish-speaking patients enabling patient empowerment through education and advocacy.

ABSTRACT

Machine learning was applied to predict evacuation rates for all census tracts affected by Hurricane Laura. The evacuation ground truth was derived from cellular telephone–based mobility data. Twitter data, census data, geographical data, COVID-19 case rates, the social vulnerability index from the Centers for Disease Control and Prevention (CDC)/Agency for Toxic Substances and Disease Registry (ATSDR), and relevant weather and physical data were used to do the prediction. Random forests were found to perform well, with a mean absolute percent error of 4.9% on testing data. Feature importance for prediction was analyzed using Shapley additive explanations and it was found that previous evacuation, rainfall forecasts, COVID-19 case rates, and Twitter data rank highly in terms of importance. Social vulnerability indices were also found to show a very consistent relationship with evacuation rates, such that higher vulnerability consistently implies lower evacuation rates. These findings can help with hurricane evacuation preparedness and planning as well as real-time assessment. © 2023 American Meteorological Society.

Individuation and liberty in a globalized world: Psychosocial perspectives on freedom after freedom ; : 73-91, 2022.

ABSTRACT

This chapter provides an overview of psychoanalytical conceptions on freedom is provided along with certain political objections. Urukagina's code has been widely hailed as the first recorded specimen of government reform, seeking to achieve a higher level of freedom and equality. The chapter encounters conical shape associated with freedom in Ancient Rome: pileus. Liberty embodies the privileges of independence, freedom, on the other hand, the rights of belonging. To top it all off, in countries with authoritarian regimes, quarantine-like regulations that are dictated by the COVID-19 pandemic created an opportunity for restricting freedom of speech and freedom of peaceful assembly. Ego is neither able to control passion nor the external world in the name of reason or freedom. Through a newly won freedom from the constraints of fantasy, the course of a psychoanalysis should enable the person to approach reality from a different perspective, to pass from the restrictions of an unconscious internal reality to the possibilities offered by whatever may happen. Consistent with his aim, Freud saw freedom as an illusion and redefined it as the recognition of necessity. (PsycInfo Database Record (c) 2023 APA, all rights reserved)

ABSTRACT

By following the 3 points below, health care leaders can improve their health care delivery practices for patients with limited English proficiency: * improve health care delivery for populations with limited English proficiency, * partner with community-based organizations that teach ESL, and * leverage Medicaid and work with insurers to better manage the care of non–English-speaking patients. _____ Coronavirus disease 2019 (COVID-19) mortality data show that Latinxs are disproportionately dying in the United States.1 These findings parallel known health disparities for Latinxs, which can be attributed in part to language barriers. When patients and their clinicians do not speak the same language, patients receive less health education and experience more testing, more readmissions, and longer lengths of stay, resulting in higher health care utilization.3 Beyond the clinical setting, language, like other social determinants of health, affects patients' lived environments, including their ability to understand food labels, apply for public housing services, and access reliable community health information. Funding could also be sought through the reintroduction of an Obama administration federal proposal that supported partnerships among states, adult education providers, higher education institutions, and private organizations.9 Bolstering language services with health-focused ESL courses may ultimately reduce health care spending and satisfy mandatory community health needs assessment implementation strategies. Health systems can then evaluate the effectiveness of their courses internally or by partnering with the Department of Education, which provides funds for evaluation of adult education programs.11 Access to health-focused ESL classes can be improved, but participation can still be hindered by day-to-day patient barriers, such as working multiple jobs and lacking transportation. [...]ESL programs should not be implemented in isolation but rather as part of a larger infrastructure focused on addressing patients' social needs.

ABSTRACT

This project explores what dreams might reveal about the collective psyche’s response to the COVID‐19 pandemic in its first year, before the development of vaccines. A brief survey, distributed to Jungian colleagues and organizations, and to various social media sites, invited people to submit online a dream related to the COVID‐19 pandemic. Four hundred and thirty‐six dreams were submitted. Forty additional Russian dreams were collected and submitted by Russian colleagues. Using qualitative research methods based on phenomenological hermeneutics, the researchers categorized and counted the range of COVID imagery. In addition, the researchers describe a range of psychic responses to the pandemic, including horror, grief, sickness, social discord, and violence, but also images of healing and transformation, increased sense of community, and spiritual renewal. Several healing nightmares are presented. Healing alchemical and anima/animus imagery is described. Twelve dreams are introduced and presented. It is concluded that the collective psyche, rooted in the Self, is a healing resource for social and cultural trauma. This project supports Beradt’s (1968) inspirational study of dreaming in Nazi Germany, as well as recent studies of COVID‐related dreams and recent publications on the social nature of dreaming. ABSTRACT

In the current article, we aim to study in detail a novel coronavirus (2019-nCoV or COVID-19) mathematical model for different aspects under Caputo fractional derivative. First, from analysis point of view, existence is necessary to be investigated for any applied problem. Therefore, we used fixed point theorem's due to Banach's and Schaefer's to establish some sufficient results regarding existence and uniqueness of the solution to the proposed model. On the other hand, stability is important in respect of approximate solution, so we have developed condition sufficient for the stability of Ulam-Hyers and their different types for the considered system. In addition, the model has also been considered for semianalytical solution via Laplace Adomian decomposition method (LADM). On Matlab, by taking some real data about Pakistan, we graph the obtained results. In the last of the manuscript, a detail discussion and brief conclusion are provided.

ABSTRACT

I present a posthumanist approach to literary interpretation using stylistic analysis. It is posthumanist since i) digital cameras/audio-video resources and editing applications prompt multimodal readings of literary works unlikely from human intuition alone;ii) anthropocentrism in literary texts is defamiliarised. I highlight how stylistic analysis can be used productively for developing multimodal creativity in posthumanist reading by motivating audio-video edits and effects. I model using Anne Brontë's poem ‘Home' (1846). When read only with intuition, ‘Home' communicates young Brontë's yearning for her family home. In contrast, this article has a non-intuitive digital multimodal realisation of this poem where a young Californian stuck in London because of pandemic (Covid-19) travel restrictions yearns for her home state in the aftermath of wildfires linked to anthropogenic climate change. This posthumanist transformative reading, flagging the negative repercussions of humans for their planetary home, defamiliarises the poem's anthropocentric normality. Importantly, I show how stylistic analysis of ‘Home' motivates creative use of audio-visual edits and effects in the posthumanist multimodal reading. The article makes contrast with standard interpretive practice in stylistics (‘humanist stylistics'). It also reflects on the value of posthumanist stylistics for extending students' creative thinking in an educational context. © The Author(s) 2023.

ABSTRACT

Pre-COVID our dermatology department had slightly fragmented and unreliable methods of storing the details of patients receiving second-line and systemic medications, such as immunosuppressive agents. Often, this information was held within individual Microsoft Excel worksheets on desktop PCs. At the onset of the COVID-19 pandemic in 2020, rapid access to this information was required to allow high-risk patients and shielding groups to be identified. We spent significant time updating and merging these separate files to form a single document. At that stage, multiuser updates and simultaneous working on a single document was not feasible, and there was often duplication and being 'locked out'. A multirow section of our newly combined document was also inadvertently deleted and could not be recovered. Thankfully, at an early stage of the COVID-19 pandemic, Microsoft 365 was rapidly adopted and rolled out in our health board. The aim was to provide a comprehensive, all-in-one digital workspace, including communications, cloud storage, backup and syncing, and productivity apps to aid working remotely and from home. Our department, in a tertiary teaching hospital, migrated to this software and it rapidly helped facilitate increased digital collaboration between clinical team members. It was recognized that its cloud storage capability would be a useful tool to help centralize and store an online, amendable patient database to record details of current and previous second- line treatments, and important information relating to shielding and COVID-19 risk status. One of the revamped application functions within Office 365 is Microsoft Lists. This offers real-time, online, secure functionality, with the ability to colour-code patients by drug, consultant and COVID-19 risk status. All members of our team have access, including administrative and clinical pharmacy staff, allowing each patient record to be easily accessed and updated. There is an additional functionality to allow email notifications of any updates (e.g. when made to specific patient entries) and even direct notification to associated stakeholders (such as government COVID-19 risk and shielding bodies). This set-up has also provided an ideal platform to allow research and clinical audit activities to be performed, which can be backwards formatted into Microsoft Excel, again for any data interpretation. Removed entries for patients no longer on second-line drugs can be automatically transferred to a linked archive list. In summary, this digital project highlights our departmental experience in using Microsoft Lists as an online, secure, cloud-based portal for patients on second-line medications and to record accurately COVID-19 risk status. It is easy to use, pleasing on the eye and its functionality could be transferrable to other clinical areas, such as in skin cancer or diagnosis logs.

ABSTRACT

Proper noun compounds, e.g., "Covid vaccine”, convey information in a succinct manner (a "Covid vaccine” is a "vaccine that immunizes against the Covid disease”). These are commonly used in short-form domains, such as news headlines, but are largely ignored in information-seeking applications. To address this limitation, we release a new manually annotated dataset, PRONCI, consisting of 22.5K proper noun compounds along with their free-form semantic interpretations. PRONCI is 60 times larger than prior noun compound datasets and also includes non-compositional examples, which have not been previously explored. We experiment with various neural models for automatically generating the semantic interpretations from proper noun compounds, ranging from few-shot prompting to supervised learning, with varying degrees of knowledge about the constituent nouns. We find that adding targeted knowledge, particularly about the common noun, results in performance gains of upto 2.8%. Finally, we integrate our model generated interpretations with an existing Open IE system and observe an 7.5% increase in yield at a precision of 85%. ABSTRACT

The global pandemic of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection has generated tremendous concern and poses a serious threat to international public health. Phosphorylation is a common post-translational modification affecting many essential cellular processes and is inextricably linked to SARS-CoV-2 infection. Hence, accurate identification of phosphorylation sites will be helpful to understand the mechanisms of SARS-CoV-2 infection and mitigate the ongoing COVID-19 pandemic. In the present study, an attention-based bidirectional gated recurrent unit network, called IPs-GRUAtt, was proposed to identify phosphorylation sites in SARS-CoV-2-infected host cells. Comparative results demonstrated that IPs-GRUAtt surpassed both state-of-the-art machine-learning methods and existing models for identifying phosphorylation sites. Moreover, the attention mechanism made IPs-GRUAtt able to extract the key features from protein sequences. These results demonstrated that the IPs-GRUAtt is a powerful tool for identifying phosphorylation sites. For facilitating its academic use, a freely available online web server for IPs-GRUAtt is provided at http://cbcb.cdutcm.edu.cn/phosphory/.

Background: To date, most countries worldwide have declared that the pandemic of COVID-19 is over, while the WHO has not officially ended the COVID-19 pandemic, and China still insists on the personalized dynamic COVID-free policy. Large-scale nucleic acid testing in Chinese communities and the manual interpretation for SARS-CoV-2 nucleic acid detection results pose a huge challenge for labour, quality and turnaround time (TAT) requirements. To solve this specific issue while increase the efficiency and accuracy of interpretation, we created an autoverification and guidance system (AGS) that can automatically interpret and report the COVID-19 reverse transcriptase-polymerase chain reaction (RT-PCR) results relaying on computer-based autoverification procedure and then validated its performance in real-world environments. This would be conductive to transmission risk prediction, COVID-19 prevention and control and timely medical treatment for positive patients in the context of the predictive, preventive and personalized medicine (PPPM). Methods: A diagnostic accuracy test was conducted with 380,693 participants from two COVID-19 test sites in China, the Hong Kong Hybribio Medical Laboratory (n = 266,035) and the mobile medical shelter at a Shanghai airport (n = 114,658). These participants underwent SARS-CoV-2 RT-PCR from March 28 to April 10, 2022. All RT-PCR results were interpreted by laboratorians and by using AGS simultaneously. Considering the manual interpretation as gold standard, the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy were applied to evaluate the diagnostic value of the AGS on the interpretation of RT-PCR results. Results: Among the 266,035 samples in Hong Kong, there were 16,356 (6.15%) positive, 231,073 (86.86%) negative, 18,606 (6.99%) indefinite, 231,073 (86.86%, negative) no retest required and 34,962 (13.14%, positive and indefinite) retest required; the 114,658 samples in Shanghai consisted of 76 (0.07%) positive, 109,956 (95.90%) negative, 4626 (4.03%) indefinite, 109,956 (95.90%, negative) no retest required and 4702 (4.10%, positive and indefinite) retest required. Compared to the fashioned manual interpretation, the AGS is a procedure of high accuracy [99.96% (95%CI, 99.95-99.97%) in Hong Kong and 100% (95%CI, 100-100%) in Shanghai] with perfect sensitivity [99.98% (95%CI, 99.97-99.98%) in Hong Kong and 100% (95%CI, 100-100%) in Shanghai], specificity [99.87% (95%CI, 99.82-99.90%) in Hong Kong and 100% (95%CI, 99.92-100%) in Shanghai], PPV [99.98% (95%CI, 99.97-99.99%) in Hong Kong and 100% (95%CI, 99.99-100%) in Shanghai] and NPV [99.85% (95%CI, 99.80-99.88%) in Hong Kong and 100% (95%CI, 99.90-100%) in Shanghai]. The need for manual interpretation of total samples was dramatically reduced from 100% to 13.1% and the interpretation time fell from 53 h to 26 min in Hong Kong; while the manual interpretation of total samples was decreased from 100% to 4.1% and the interpretation time dropped from 20 h to 16 min at Shanghai. Conclusions: The AGS is a procedure of high accuracy and significantly relieves both labour and time from the challenge of large-scale screening of SARS-CoV-2 using RT-PCR. It should be recommended as a powerful screening, diagnostic and predictive system for SARS-CoV-2 to contribute timely the ending of the COVID-19 pandemic following the concept of PPPM.

ABSTRACT

Wellens syndrome is a precursor of left anterior descending (LAD) coronary stenosis. It is characterized by biphasic T waves in V2-V3 (type A) or negative deep T waves in V2-V4 (type B). The ability of emergency physicians, hospitalists, or primary care providers to recognize these early ECG patterns is primordial because the definitive treatment is urgent cardiac catheterization with percutaneous coronary intervention. However, failure to identify a type A or type B Wellens syndrome may lead to devastating outcomes, such as myocardial infarction or even death. We presented a clinical case of Wellens' syndrome with deep T waves in V2-V3 associated with COVID pneumonia, pleural effusions, and congestive heart failure that went to a rapid and massive myocardial infarction.

ABSTRACT

OBJECTIVES: The aim of this study was to compare screening mammography performance metrics for immediate (live) interpretation versus offline interpretation at a cancer center. METHODS: An institutional review board-approved, retrospective comparison of screening mammography metrics at a cancer center for January 1, 2018, to December 31, 2019 (live period), and September 1, 2020, to March 31, 2022 (offline period), was performed. Before July 2020, screening examinations were interpreted while patients waited (live period), and diagnostic workup was performed concurrently. After the coronavirus disease 2019 shutdown from March to mid-June 2020, offline same-day interpretation was instituted. Patients with abnormal screening results returned for separate diagnostic evaluation. Screening metrics of positive predictive value 1 (PPV1), cancer detection rate (CDR), and abnormal interpretation rate (AIR) were compared for 17 radiologists who interpreted during both periods. Statistical significance was assessed using χ2 analysis. RESULTS: In the live period, there were 7,105 screenings, 635 recalls, and 51 screen-detected cancers. In the offline period, there were 7,512 screenings, 586 recalls, and 47 screen-detected cancers. Comparison of live screening metrics versus offline metrics produced the following results: AIR, 8.9% (635 of 7,105) versus 7.8% (586 of 7,512) (P = .01); PPV1, 8.0% (51 of 635) versus 8.0% (47 of 586); and CDR, 7.2/1,000 versus 6.3/1,000 (P = .50). When grouped by >10% AIR or <10% AIR for the live period, the >10% AIR group showed a significant decrease in AIR for offline interpretation (from 12.7% to 9.7%, P < .001), whereas the <10% AIR group showed no significant change (from 7.4% to 6.7%, P = .17). CONCLUSIONS: Conversion to offline screening interpretation from immediate interpretation at a cancer center was associated with lower AIR and similar CDR and PPV1. This effect was seen largely in radiologists with AIR > 10% in the live setting.
